What you’ll do

If you’re ready to jump into the administrative side of nursing, this position is for you! We will count on you to complete all referrals by accessing the hospital EMR to gather H&P, F2F, and other relevant documentation. You’ll also work with our Central Intake Department when needed. You’ll also work with schedulers and the medical records team to coordinate the needs of all team members and patients. Success in this role is all dependent on your ability to work and collaborate with your entire team, and we know you can do it!

 

What it takes

  • Current Ohio nursing license to practice as a Licensed Nurse (LPN) required
  • One year experience in a home health, hospice, or long-term care healthcare setting required
  • One year experience in a Medicare-certified home health agency required
  • Valid driver’s license, insurance, and acceptable driving record
